• Families who want to protect what they’re able to secure and what they already have in place for their relative – this is crucial and could save their relative from having to move out of and sell their home due to a lack of suitable Care and Support.
• Anyone who wants to ensure that their relative continues to have control over their own life.

Key Deliverables

• Meeting to firmly establish the funding responsibility of the Local Authority and what’s currently delivered / offered vs what’s actually required.
• A thorough review of housing, care and support options, suggestions and recommendations, including maintaining home ownership.
• ‘Hand holding’ to help deliver outcomes from an experienced Care and Support professional.
• Accompanying the individual, their family and support worker to an assessment if required.

Cost

• £450 - £950 this includes the cost of a Care and Support professional based on the individual’s requirements.
